You may send the form back to NYSDOL via your online account, by fax, or by mail. Follow the instructions on the bottom of the form.
Once NYSDOL receives your completed Request for 1099-G Review form, it will be reviewed, and we will send you an amended 1099-G tax form or a letter of explanation.

What To Do If You Lost Your W
The W-2 form is a vital piece of info for most tax filers as it confirms the income you earned for that tax year. Sometimes known as the Wage and Tax Statement, this is sent out by employers at the beginning of every calendar year.

You can file your federal tax return without a 1099-G form as long as you are informed of:
- The full amount of unemployment benefits paid to you during the previous calendar year.
- The number of federal taxes withheld, if any, during the previous year.
You dont have to attach the 1099-G form to your federal income tax return. If it is essential to file a state income tax return, you may need to attach a copy of the 1099-G to your state return. Review your countrys instructions on how to fill the state income tax return.

Federal Income Taxes On Unemployment Insurance Benefits
Although the state of New Jersey does not tax Unemployment Insurance benefits, they are subject to federal income taxes. You can opt to have federal income tax withheld when you first apply for benefits. After each calendar year during which you get Unemployment Insurance benefits, we will provide you with a 1099-G form that shows the amount of benefits you received and taxes withheld. This information is also sent to the IRS.

WASHINGTON With millions of Americans now receiving taxable unemployment compensation, many of them for the first time, the Internal Revenue Service today reminded people receiving unemployment compensation that they can have tax withheld from their benefits now to help avoid owing taxes on this income when they file their federal income tax return next year.
Withholding is voluntary. Federal law allows any recipient to choose to have a flat 10% withheld from their benefits to cover part or all of their tax liability. To do that, fill out Form W-4V, Voluntary Withholding RequestPDF, and give it to the agency paying the benefits. Don’t send it to the IRS. If the payor has its own withholding request form, use it instead.
If a recipient doesn’t choose withholding, or if withholding is not enough, they can make quarterly estimated tax payments instead. The payment for the first two quarters of 2020 was due on July 15. Third and fourth quarter payments are due on September 15, 2020, and January 15, 2021, respectively. If I Repaid An Overpayment Will It Be Reflected On My 1099
No. DES reports the total amount of benefits paid to you in the previous calendar year on your 1099-G, regardless of whether you repaid any overpayment. If you repaid part or all of an overpayment during the previous calendar year, you may be able to deduct the repaid amounts on your income tax return. The repaid amount should be reported on the tax return submitted for the year the repayment was made.
Information Needed For Your Federal Income Tax Return
Each January, we mail an IRS Form 1099-G to individuals we paid unemployment benefits during the prior calendar year. The 1099-G form provides information you need to report your benefits. Use the information from the form, but do not attach a copy of the 1099-G to your federal income tax return because TWC has already reported the 1099-G information to the IRS.
