To access your Form 1099-G, log into your account at labor.ny.gov/signin. Click the Unemployment Services button on the My Online Services page. Click the Get Your NYS 1099-G button on the Unemployment Insurance Benefits Online page. If you prefer to have your Form 1099-G mailed to you, you can call 1-888-209-8124.
Disagree With Your 1099
If you disagree with any of the information provided on your 1099-G tax form, you should complete the Request for 1099-G Review.
You may send the form back to NYSDOL via your online account, by fax, or by mail. Follow the instructions on the bottom of the form.

What Is A 1099
The 1099-G is a tax form for Certain Government Payments. Every January, we send a 1099-G form to people who received unemployment benefits during the prior calendar year. If you received unemployment benefits during 2021, youll need the information to file your taxes. Claimant tax information cannot be shared over the phone and claimants are not able to access this information by calling the UI Claimant Assistance Center.
NOTE: Claimants will likely receive multiple 1099-G forms for 2021. This is due to receiving benefits from multiple programs. Those programs that claimants would receive a 1099-G form form include:

How To Get Your 1099
DENVER Many who received unemployment benefits last year are now depending on tax returns but some are still waiting to get their 1099-G forms from the Colorado Department of Labor and Employment .

The CDLE explains all PUA-related 1099-G forms can be accessed on a claimants MyUI+ account. If a claimant received PUA benefits as well as regular state unemployment benefits, PEUC, or SEB, the 1099 in their MyUI+ account will only reflect the PUA benefits.
If a claimant did not receive their 1099 in the mail, they can complete this form to receive another copy in the mail. Claimants should also complete this form if they need another copy of the 1099 that reflects all non-PUA benefits they were paid in 2020. If a claimant only received PUA benefits in 2020, they should not complete the form and instead access the 1099 via MyUI+.
