You may request duplicate W-2’s for the most recent calendar year beginning February 15.
1. The DTMB-Office of Financial Management handles the duplicate W-2 and 1095-C requests. Please call the Duplicate W-2 and 1095-C Request Line at 517-241-0045, and leave a voice message providing the following information:
- Last 4 digits of your Social Security Number
- A daytime telephone number where you can be reached
- Year duplicate W-2 is needed
- Current Mailing Address
- State Government Agency
NOTE: If your need for a duplicate W-2 arises as a result of a change of address you will need to make sure that your new mailing address is correct in HRMN before requesting a duplicate W-2 . You may review and update your address in HRMN via your online employee self-service account or by contacting MI HR Service Center at 877-766-6447 .

Once your request has been received, you may expect to receive your duplicate W-2 within 2 weeks.
Request Your Unemployment Benefit Statement Online
Because unemployment benefits are taxable, any unemployment compensation received during the year must be reported on your federal tax return. If you received unemployment benefits in 2020, you will receive Form 1099-G Certain Government Payments .
The statements, called 1099-G or âCertain Government Payments,â are prepared by UIA and report how much individuals received in unemployment benefits and income tax withheld last year.
You can choose to receive your 1099-G, electronically through MiWAM or by U.S. mail.
To receive your 1099-G electronically, you must request your delivery preference by January 9, 2021. Your statement will be available to view or download by mid-January. If you do not select electronic, you will automatically receive a paper copy by mail.
To receive your 1099-G online:

2 Under Account Alerts, click Please select a delivery preference for your 1099 Form
3 Under Delivery Preference for Form 1099-G, click Electronic. Your email address will be displayed.
4 â Review and Submit. You will receive an email acknowledging your delivery preference.
To receive your copy by mail, follow the steps above and select paper as your delivery preference.

These Are The States That Will Not Mail You Form 1099
- To access your Form 1099-G, log into your account through at uinteract.labor.mo.gov. From the UInteract home screen, click View and Print 1099 tab and select the year to view and print that years 1099-G tax form.
- The Missouri Division of Employment Security will mail a postcard no later than January 31, 2021, notifying anyone who has not accessed their Form 1099-G online about the availability of the form and how to access it.
- To access your Form 1099-G, check your email. You will receive your Form 1099-G by email. You can also use the Check Claim Status tool to get your Form 1099-G.
- If you prefer to have your Form 1099-G mailed, you may request a copy from your Reemployment Call Center. It may take 10 business days to receive a copy of your Form 1099-G.
- To access your Form 1099-G, log into your account at labor.ny.gov/signin. Click the Unemployment Services button on the My Online Services page. Click the Get Your NYS 1099-G button on the Unemployment Insurance Benefits Online page.
- If you prefer to have your Form 1099-G mailed to you, you can call 1-888-209-8124. This is an automated phone line that allows you to request to have your Form 1099-G mailed to the address that you have on file.

How Can I Download My 1099
If you were out of work for some or all of the previous year, you arenât off the hook with the IRS. Those who received unemployment benefits for some or all of the year will need a 1099-G form. Youâll also need this form if you received payments as part of a governmental paid family leave program. But you donât have to wait for your copy of the form to arrive in the mail. How To Claim Unemployment Benefits
Each state has its own guidelines for how to claim unemployment benefits. There are also, typically, requirements you must follow to continue receiving the benefits.
The first thing to do is gather the documents you will need to file your claim. This is because when you file a claim, your states unemployment insurance agency will ask you for details around your former employment, such as addresses and dates. You should take the time to provide the most complete and accurate information you can, as it lessens the chances of your claim being delayed.

What To Do If You Cant Find Your W 2 Form
The W 2 form is usually mailed to you or made accessible online by the company you work for. If the form is lost, missing, or you cant find it online, contact your employer immediately. Its a good idea to call the IRS if you dont receive the W2 by mid-February.
In the worst-case scenario, you can fill out Form 4852 and attach it to your tax return. The substitute wage and tax statement allows you to input the same details. A tax extension is another possibility and buys you more time.
Your best bet is to file on time. If the W 2 arrives later, and the information is different from your estimates provided, Form 1040X can be filled out to correct any discrepancies, and amend the previous years tax return.

Update 12/29/20: The federal government has extended the federal unemployment benefit programs available through the CARES Act for an additional 11 weeks. Pandemic Unemployment Assistance and Pandemic Emergency Unemployment Compensation will be available through March 14, 2021, with qualified claimants benefits fully phasing out by April 5, 2021. Who Can Get Their W2 Online
Many companies, including the military, have their employees W-2s available online for free. The documents can be downloaded via a variety of outsourcing companies, so the distribution process of the W-2s are automated. Your employer should alert you as to whether or not you will be able to access your W2 online.
If they do allow you to access it online, you will get the benefit of being able to file your taxes earlier in addition to not waiting for paper copies, and eliminating the security risks that come along with having that information sitting around on paper.

What Is A 1099
The 1099-G is a tax form for Certain Government Payments. Every January, we send a 1099-G form to people who received unemployment benefits during the prior calendar year. If you received unemployment benefits during 2020, youll need the information to file your taxes. Claimant tax information cannot be shared over the phone and claimants are not able to access this information by calling the UI Claimant Assistance Center.
NOTE: Claimants will likely receive multiple 1099-G forms for 2020. This is due to receiving benefits from multiple programs. Those programs that claimants would receive a 1099-G form form include:
