If youve been laid off or become unemployed, you may be able to continue with your employers health insurance under the Consolidated Omnibus Budget Reconciliation Act, better known as COBRA. Assuming you qualify, you could stay covered by your employers group plan for up to 18 months after leaving your job.

Thats great news if you were happy with your employers insurance,, but theres a catch: COBRA coverage shifts the responsibility for paying premiums to you and its possible that you may be required to pay the entire amount due, up to 102% of the cost to the plan.

If youve lost your job and dont have another position lined up right away, its possible that COBRA coverage isnt the best route for your budget. but you do have a bit of time to decide. Normally, youd have 60 days after losing your employers coverage to elect COBRA coverage. Because of the coronavirus pandemic, however, you now have until 60 days after the COVID-19 national emergency ends to decide. This is an advantage as it means you have more time to weigh other -potentially less expensive health insurance options before making a full commitment.

Medicaid Expansion For The Unemployed

Under the Affordable Care Act, states have the option to expand Medicaid to families with incomes up to 138 percent of the federal poverty level. To rapidly absorb the unemployed into affordable coverage, states should have the option to further expand Medicaid to two categories of enrollees: 1) individuals who are receiving unemployment insurance benefits or who were receiving unemployment insurance benefits that expired during the public health emergency and 2) families with incomes up to 200 percent of the federal poverty level. As under the current Medicaid program, eligible individuals who are enrolled in ACA coverage or employer coverage would be able to switch to Medicaid.

For the first eligibility category, states would automatically enroll in Medicaid all individuals who receive unemployment insurance benefits or food stampsunder the Supplemental Nutrition Assistance Program regardless of their income. This auto-enrollment would be similar to the current Express Lane Eligibility option, in which several states enroll children in Medicaid or the Childrens Health Insurance Program based on their SNAP eligibility.

States would guarantee continuous eligibility for these enrollees until the end of the public health emergency, at which point their eligibility would be redetermined. If they are no longer receiving unemployment insurance benefits, they would qualify for Medicaid under the second eligibility category.

If You Qualified For Cobra Premium Assistance

If you qualified for COBRA continuation coverage because you or a household member had a reduction in work hours or involuntarily lost a job, you may have qualified for help paying for your COBRA premiums .

COBRA premium assistance under the American Rescue Plan Act of 2021 was available April 1, 2021 through September 30, 2021. Since this help ended on September 30, you can enroll in a Marketplace plan with a Special Enrollment Period. To enroll, you can report a September 30 “loss of coverage” on your application. You cant qualify for a premium tax credit while youre enrolled in COBRA, so if you want to change to Marketplace coverage, make sure that your COBRA coverage ends on the last day before your Marketplace coverage can start. If you decide to keep COBRA without premium assistance, you can qualify for a Special Enrollment Period based on the end date of your COBRA coverage, which is usually 18 to 36 months after it started.

Hours of insurable employment are the hours you work, for one or more employers under written or verbal contracts of service, for which you receive wages.

The qualifying period is the shorter of:

What Youll Pay Each Month

The unemployment-based subsidies will cover the full cost of the benchmark plan. So youll have access to two Silver plans that have no premium, and youll likely have access to a variety of Bronze plans and possibly some Gold plans that have no premium.

If you pick a plan thats more expensive than the benchmark plan, including the higher-cost Silver plans, youll pay at least some premium each month.

If youre in a state that has additional state-mandated benefits that arent covered by premium subsidies, you may find that you have to pay at least a dollar or two each month in premiums, regardless of which plan you select.

Do I Need Health Insurance Between Jobs

Having health coverage when youre in between jobs can help protect you should you need care. If you need medical care, a health insurance plan can provide coverage for eligible services. That means once you meet your deductible, you and your plan share the costs for your care. And under most plans your preventive care is covered at 100%. Without health insurance, you may have to pay out-of-pocket for any care you receive.

Choosing A Marketplace Plan

Within the marketplace, you can choose different types of health insurance plans designated as bronze, silver, gold, and platinum based on the level of coverage:

  • Bronze plans offer the lowest monthly premium but highest costs when you need care, with deductibles that can be thousands of dollars per year. These are best for low-cost coverage against worst-case medical scenarios.
  • Silver plans offer more moderately priced premiums and costs when you need care with deductibles lower than Bronze plans. Silver plans are best if you qualify for a cost-sharing reduction or are willing to pay a slightly higher premium in return for lower costs when you receive care.
  • Gold plans have higher monthly premiums than either silver or bronze plans but provide more help paying for the care you receive with fairly low deductibles. If you receive a lot of medical care, these can be good options.
  • Platinum plans carry the highest monthly premium and provide the lowest costs when you receive care with very low deductibles so your plan starts paying earlier than other categories of plans. If you receive a lot of medical care and dont mind paying a high monthly premium to have almost all of your other costs covered, platinum plans may be a good option.

You may have access to catastrophic plans with low monthly premiums, but very high deductibles and are designed to protect you from worst-case scenarios.
