Filing For Weekly Benefits
Once youve filed your initial claim for unemployment benefits, you can access your account each week and apply for benefits, check on the status of the claim to see when payment was made, and review how much unemployment compensation is left in your account. Mark your calendar with the day you need to file. Payment won’t be made unless you file for benefits each week for which you’re eligible.
While receiving unemployment, you must be actively willing to work and actively looking for work. The definitions of actively willing to work and actively looking for work also vary by state.
Information Required To Apply For Unemployment
If youve been laid off from your job, the chances are high that you will qualify to file for unemployment benefits.
In most states, unemployed workers can apply online, over the phone, or, in some cases, by mailing a form. Many states provide information for applicants in numerous languages, including Spanish.
Applying online is the quickest and easiest way to file for unemployment. Claims are generally processed much more quickly, so you stand to start receiving benefits sooner than applying by phone or mail.

How Do I Apply
To receive unemployment insurance benefits, you need to file a claim with the unemployment insurance program in the state where you worked. Depending on the state, claims may be filed in person, by telephone, or online.

These forms arrive when a former employee has been separated from their next employer. The form appears to be informational in nature, and can list one or more former employees on it. Sometimes employers read it, and file it away, or toss it out.
In actuality, this form is notifying you that a former employee has filed for unemployment benefits and that your account will be charged, unless you respond within 30 days.
The Notice to Base Year Employer form shows:

Even though it comes with no form to fill out or questions to answer, you need to provide the same information that you would on a Separation Statement questionnaire. You can do this via a letter, a fax, or by writing on the form and returning it.
